League of Legends: Beating The Revive Meta

You may have noticed that there are now a few champions roaming around with the ability to, lets say, "Cheat Death". It's okay knowing which champions are capable of doing this, but how do you combat it? How can you make it easier on yourself and stay alive long enough to make sure these guys go down and stay down? What happens if they suddenly decide to buy a Guardian Angel? You'd have to kill them three times!! They may not have full health the second and third time, but still, 3 times is too many.
There are ways to combat their passives, but doing so isn't always easy. For example:
Zac's passive "Cell Division" splits him into 4 bloblets, each with 12% of his maximum health, for a whole 8 seconds, meaning you have plenty of time to beat them down. Depending on how many bloblets are left over, he can reform and heal for 10-50% of his max health, which can be a lot. You can hit Zac with grievous wound passives from items, which can reduce his healing potential as a whole, but it will not affect his bloblets and revive. It has a 300 second cooldown, so it shouldn't get in the way too much. A well placed strong AoE attack can destroy all of the blobs at the same time, keeping him down for the count.
Anivia was the first revivable champion and her system is pretty good. For her passive; "Rebirth", she turns herself into an egg for 6 seconds with an armor and magic resistance modifier of −40 / −25 / −10 / +5 / +20. She is unable to make any action what-so-ever and will have to sit there whilst you punish her. She doesn't generally have a lot of health, so her egg isn't particularly hard to take down. It has a 240 second cooldown and as a whole, isn't much trouble to deal with, but I would consider it to be the most fair.
Now we have Aatrox. His passive; "Blood Well", is better than the other two in 1 aspect; it makes him invulnerable and untargetable for 3 seconds whilst he heals. In a sense, this can be deemed as overpowered or OP for short, but there are a few negatives to his passive that you need to be aware of. The first thing is that if he isn't casting abilities that cost health on a regular basis, his blood well will run dry, meaning that if he manages to revive, he will only have a portion of what he is capable of getting. After 5 seconds of not dealing damage (this includes auto-attacks), it depletes by 2% per second.
He can contan a maximum of 30 + (45 × level), and heal for that, plus 10.5 + (15.75 × level) so really he will never really be able to gain over half of his natural health until a really high level. In the laning phase, this might be a problem to stand up against, but in a team fight, this will be harder for him to pull off efficiently, especially if he is being slammed with CC (crowd control) and being unable to deal much damage, or just generally being focused down. The cooldown for this passive is 225 seconds
