How to Cause Teemo Infinite Pain

As the most hated champion in the League of Legends, Teemo has become the most celebrated one to kill. There are a few ways to go about it, from itemization to champion select.
Champions
If you're in draft mode ranked or normal and see a Teemo pick lock in, he's probably going top or maybe mid. If you're aiming for those roles, you'll want to pick a champion with a gap closer. Within the 114 total released, 12 of the last 15 have gap closers, so you'll have no trouble picking one up that's to your preferences. In the mid lane, (staying with the last 15 champions to keep it simple) Lissandra and Zed both have abilities that allow them to get up close and personal.
Teemo does have a movement speed increase with his W 'Move Quickly,' but it can get him only so far if you're right on his tail and laying down crowd control on top of that. Lissandra's E 'Glacial Path' throws out a large claw that deals magic damage. If activated again at any point before it stops, Lissandra can blink to it and use any of her other abilities to keep Teemo within range and under pressure.
Zed's W 'Living Shadow' spawns a shadow that mimics Zed's abilities and can be blinked to for 4 seconds. More specifically, it can mimic his E 'Shadow Slash' which slows enemies hit by it. The strategy there would be to throw the shadow out at Teemo and activate his E to slow him down, and by activating his W again, Zed is right next to him ready to dole out more damage.
Zed is slightly different in his case as he is an AD caster and his high damage auto attacks can be blinded by Teemo, however it's times like these that we need to go into man mode and deal with it. We cannot afford to let him survive another game.
At top lane, the champion Aatrox can use his Q 'Dark Flight' to help kill Teemo. The typical Teemo strategy is hit and run, so Aatrox can give chase and try to get him to activate his W. Once he does, using Dark Flight and knocking Teemo up will waste the timer and he'll have a much harder time getting away.
Once Dark Flight has been landed, Aatrox can use his E 'Blades of Torment' to slow Teemo and deftly finish him off. Once again, a Teemo running away is only likely to shoot his blinding dart, but not stick around and try for more damage unless he's baiting a gank.
Yet another recent champion with a gap closer that has been released recently is Kha'Zx. His E 'Leap' allows him to jump to a target area to deal area of effect physical damage. Once up in Teemo's face, Kha'Zix can throw out his W 'Void Spikes' to heal himself, hurt Teemo, and slow him down to stay within autoattack range. Although Teemo can blind him, Kha'Zix has the abilities to deal damage anyway.
When activated, his Q 'Taste Their Fear' deals physical damage to a single target and when Teemo is alone (running away) it deals an extra 45% damage. So even if blinded, Kha'Zix can use his Q while he waits for the opportunity to autoattack.
Items
If you're not able to play any of the champions listed above (there are many others) you can always pick up certain items that'll help you to eliminate the threat of Teemo's mushrooms. At the most basic level, Oracle's Elixir can be bought in Summoner's Rift. When bought, it reveals any hidden champions or abilities like Teemo's passive 'Camouflage' and his R 'Noxious Trap.'
Many Teemos utilize his passive ability that grants him 40% increased attack speed after spending time invisible. If he lays a mushroom down nearby and lie in wait, the amount of burst damage can be incredible especially if you're slowed. Buying an Oracless or buying Vision wards can keep you and your teammates safe and less likely to be caught. On The Twisted Treeline, Crystal Scar, and Howling Abyss, the equivalent to Oracle's Elixer is Oracle's Extract, which has the exact same 5 minute long stealth detection but costs 150 less gold than on Summoner's Rift.
On the non-Summoner's Rift maps, AD Teemo-haters can buy The Lightbringer. It grants 50 attack damage, 20 armor and 12% lifesteal. One of its unique passives 'Trap Detection' acts as an Oracle's, but only applies to traps like Shaco's boxes or Teemo's mushrooms. Its unique active however, lays down a stealth detecting mist that finds any traps as well as stealthed champions.
So as a bruiser or AD carry, you're able to fearlessly traverse bushes and chokepoints and continue to shred your opponents. For those who find themselves playing a champion centered around ability power, the Hextech Sweeper provides 50 ability power and 300 health as well as the exact same unique passive and active. While spending gold on one of those the team would be fine, two would completely shut down Teemo's devlish antics.
